In patients with microsatellite stable disease (pMMR\/MSS), total neoadjuvant therapy (TNT) is now considered the new standard of care.<\/p>\n<p>However, there is currently no uniform standard in Austria for the implementation of TNT in locally advanced rectal cancer (LARC). There is broad consensus that total neoadjuvant therapy represents the preferred treatment strategy for locally advanced or distally located rectal cancer; however, the specific TNT protocol selected may vary between institutions. The used protocols may differ in several aspects, such as the timing of chemotherapy (induction or consolidation chemotherapy), the radiotherapy approach (short-course radiotherapy or long-course chemoradiotherapy), the number of chemotherapy cycles, and the agents used (CAPOX, FOLFOX, FOLFIRINOX, etc.). Current clinical guidelines also do not provide specific recommendations as to which TNT protocol should be preferred.<\/p>\n<p>In patients with MSI-H\/dMMR disease, immunotherapy is on the verge of becoming the new standard treatment. This non-interventional study (NIS) will also focus on the clinical implications of the transition from conventional treatment forms of therapy (chemotherapy, radiotherapy, and surgery) to immunotherapy.<\/p>\n<p>The goal of this observational study is to build a disease-specific registry aimed at assessing the therapeutic landscape of patients with locally advanced rectal cancer treated with TNT or dMMR\/MSI-H patients in Austria<\/p>\n<h3>Primary Objective:<\/h3>\n<ul>\n<li>Recording of treatment responses according to clinical standard therapy<\/li>\n<\/ul>\n<h3>Secondary Objectives:<\/h3>\n<ul>\n<li>Overview of different TNT concepts used<\/li>\n<li>Recording of participants using watch &amp; wait therapy<\/li>\n<li>Recording of participants using immunotherapy<\/li>\n<li>Overview of the used surgical techniques<\/li>\n<li>Recording of locoregional recurrences<\/li>\n<li>Recording of the occurrence of distant metastases<\/li>\n<li>Assessment of event-free survival (EFS)<\/li>\n<li>Assessment of TME-free survival in watch &amp; wait group<\/li>\n<li>Assessment of overall survival (OS)<\/li>\n<li>Assessment of locoregional event-free survival (LREFS)<\/li>\n<li>Assessment of distant metastasis-free survival (DMFS)<\/li>\n<li>Overview of Charlson Comorbidity Index (CCI) Score prior to any treatment start<\/li>\n<li>Overview of ECOG performance status at diagnosis<\/li>\n<li>Overview of resection rates<\/li>\n<li>Overview of treatment adherence<\/li>\n<\/ul>\n<h3>Exploratory Objectives:<\/h3>\n<ul>\n<li>Overview of the biology, prognosis, diagnostics, therapy, and predictive factors of rectal cancer and related malignancies (e.g., colon cancer)<\/li>\n<li>Overview of molecular parameters (e.g., KRAS, NRAS, BRAF, HER2, microsatellite status)<\/li>\n<li>Overview of available MR\/CT\/PET-CT imaging data for translational research<\/li>\n<li>Assessment of MRI-Response using machine-learning before and after total neoadjuvant therapy<\/li>\n<li>Overview of available endoscopy imaging data for translational research<\/li>\n<li>Overview of available surgical information data for translational research<\/li>\n<li>Overview of available radiation treatment data for translational research<\/li>\n<li>Overview of available pathology data for translational research<\/li>\n<li>To study additional research questions \/ to perform additional analyses on biological material and \/ or clinical data that have been collected within clinical routine.<\/li>\n<\/ul>\n<h3>Patient Population:<\/h3>\n<p>Participants with locally advanced adenocarcinoma of the rectum treated with total neoadjuvant therapy or dMMR\/MSI-H disease<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Treatment: 33\u201350% of colorectal cancers (CRC) are localized in the rectum.
